This form has reopened, and will be open to submit proof of vaccination until Monday, September 13th at 11 pm.

A reminder: EVERYONE 12 and older who attends, inside or outside, MUST be vaccinated; PROOF of vaccination is required to ENTER the building.

Please use this form to upload proof of being fully vaccinated for you and your family to be added to our list; you need only do this once.

"Fully vaccinated” means your final vaccine dose was at least 2 weeks prior to your attendance at services. Proof consists of your vaccination card, which we ask you to scan/photograph and upload below.

There will be no exceptions to this deadline; after the deadline, you will need to bring your card with you if you wish to enter the building. This form is now open for Yom Kippur and future services. We will compile an ongoing list of all those who have submitted documentation. Those on the list and those presenting documentation in person will be allowed in the building. If you are unable to be vaccinated for medical reasons, please reach out so we can make accommodations.
